Loss Prevention Bulletin Issue 308

We are pleased to announce that the latest issue of the Loss Prevention Bulletin (LPB) has now been published to mark the 40th anniversary of the Chernobyl disaster.
This issue of the Loss Prevention Bulletin brings together its usual diverse collection of case studies, technical analyses, and lessons learned in the process safety field.
It opens with reflections on a visit to the Chernobyl site, followed by a feature article examining how nuclear safety has advanced since 1986, while emphasising the ongoing need for vigilance and international cooperation. This feature article is free to download.
The edition also introduces a new monograph series, Fifty Years of Learning: A Process Safety Monograph Series, which will publish a set of standalone instalments exploring recurring themes from a lifetime in process safety — from HP/LP interfaces and loss‑of‑containment mechanisms to human factors, layout, design philosophy, and emerging risks in the energy transition. Part 1 of the series, included in this issue, focuses on high‑pressure/low‑pressure interfaces and reflects on this persistent hazard.
Further contributions include an examination of critical roles in organisational change risk assessment, and a learning article on liquid hammer at the Hasdrubal terminal, highlighting opportunities to strengthen design and operational resilience. Another case study explores amine stress corrosion cracking of carbon steel pipework, offering practical insights for corrosion management.
In the area of process safety practice, the issue features a paper on iterating, integrating, and improving human factors at two COMAH upper‑tier sites, demonstrating how sustained attention to human performance can enhance overall risk control.
The issue also includes an obituary for long‑time LPB contributor Basil Eddershaw, with a tribute from Peter Ball.
