Richard is a Chartered Occupational Psychologist, a Registered Ergonomist and a Director of The Keil Centre. He leads The Keil Centre’s human factors team of consultants.

Richard has many years’ experience gained in defence, aviation and in the process industries. His experience covers all aspects of human factors, particularly human safety analysis (including human error) and the design and evaluation of equipment to meet user requirements.

Richard holds a BSc and MSc in Applied Psychology and along with his colleague Ronny Lardner won the British Psychological Society DOP Practitioner of the Year award in 2006 for the development of the Human Factors Analysis Tools.

Richard's talk will examine the latest developments in incident investigation guidance.

What does a good, in-depth investigation look like?
This plenary session will draw on lessons from a broad range of industry partners, including organisations dedicated solely to investigations, to explore what investigation best practice looks like.

It will place particular emphasis on understanding the behavioural root causes of process safety events.

The session will make the case for integrating human factors throughout investigations—not only during data collection and analysis, but also in the way the overall investigation process is managed.

This approach helps ensure that behavioural and human factors causes are properly considered, that investigation outputs lead to effective actions, and that lessons learned are shared in a meaningful way.

These lessons may lead to targeted changes in processes that contributed to human performance falling below expectations, as well as broader changes that support cultural improvement across an organisation.
